The announcement by India with regard to drug distribution policy and
Ted Lankester's comment about drug overuse underscore a crucial point
in any health promotion policy for low income settings.

That crucial point is the need to appropriately train local (village)
health care workers as part of an overall policy of health systems
strengthening. The record of good success in the training of village
health care workers contributes not only to a more careful and
rational use of medications, it also promotes illness-prevention and
lowers the incidence and demand for such drugs in the first place.

It is hard to overuse when one does not need to use at all.
