* at the macro-level: through public policy and equitable resource
allocation processes;
* at the meso- level (the community): through performance of the
decentralized policy;
* at the micro-level: through factors related to the organisation of the
health care system (physical, financial, psycho-social, cultural and
administrative access) as well as factors related to the health care
provider (skills, knowledge, approach to the patient). This is where
primary health care systems may contribute to comprehensive care...."
